Request for GPS Topo Map Refund
Print this form and mail or fax per the instructions below.

A			Purchased Retail GPS Maps with a CD/DVD
			Sign lines C1 through C6 below and return original CD/DVD and case to the address on the Retail Order Form. Fill in the address portion for the return check. Payment will be made in 4-6 weeks. I understand that I will only receive a refund for the purchase price, no shipping or sales tax.

B			Purchased Downloaded StateTopos.EXE file, no CD/DVD
			Sign lines C1 through C5 below and send it along with a printed copy of your email download authorization. Fax both copies to (815) 642-0947 or mail both copies to the retail mailing address. Your refund will be made in the same form as the original payment in 2-4 weeks.

C	1	______________	I have deleted all copies of the file **_______Topos.EXE from any and all hard drives.
	2	______________	I have destroyed all backup copies of **_______Topos.EXE on any media type.
	3	______________	I have not given or sold any copies of **_______Topos.EXE to any other individual or group.
	4	______________	I have deleted all copies of the *_______Topos.img files° from any and all hard drives.
	5	______________	I have deleted all GPS Topo map copies inside any and all GPS receivers and/or flash cards.
	6	______________	I have destroyed any and all backup copies of any and all files on the CD/DVD that were made.

			°All .img files were installed by StateTopos.exe into C:\Garmin\Maps\State*Topos\. by default, you may have entered an alternate drive letter and/or folder location during the custom install.

			All refunds must be requested within 6 weeks of the original purchase invoice date or download email.
